    Common Signs for Referrals

    • Marked fall in school or job performance 

    • Poor grades in school despite trying hard 

    • Severe worry or anxiety (Refusal to go to school or take part in activities that are normal for child’s age) 

    • Frequent physical complaints (headaches, stomachaches, etc.) 

    • Changes in sleeping or eating habits (Falling asleep in class often, skipping lunch, etc.) 

    • Difficulties concentrating 

    • Sexual acting out 

    • Prolonged negative mood or attitude 

    • Isolating themselves from others 

    • Changes in mood 

    • Excessive worry or fear 

    • Repeated use of drugs or alcohol 

    • Changes in sleeping or eating habits (Falling asleep in class often,  work attendance issues, inability to maintain employment, skipping lunch, etc.) 

    • Relationship issues/ volatile relationships 

     

    Ways Case Managers can help

    •  Parent Education

    •  Advocacy and Empowerment Education

    •  Psychoeducation and Coping Skills Education

    •  Assertiveness Training

    •  Social Skills Training

    •  Resource Education

    •  Community Meeting Attendance 

      • IEP meeting, school team meeting, Wraparound meeting, etc

    •  Collateral Consultation with parent, teacher, treatment provider, or community member

    •  Teach skills to access needed services, support systems, and resources

    •  Community Resources/ Referrals

      •  Food pantry, housing, medical, insurance, etc

    ***Catalyst Counseling Case Managers are not permitted to transport anyone, at any time.
    ***Case Managers can provide services to medicaid students only.
