The Board of Education for Franklin City Schools will move the dates of its already scheduled regular meetings on April 8 and June 24, both of which were to be held at 6 p.m. These rescheduled regular meetings will now be scheduled as special meetings for April 11 and June 17 at 6 p.m. These meetings will still be held in the Franklin Junior High School Cafeteria on 750 E 4th St, Franklin, OH 45005. The purpose of these meetings is to hold a rescheduled regular meeting for general purposes.

The Board of Education for Franklin City Schools will hold a special meeting on April 17 at 5:00 p.m. in the Franklin Junior High School Cafeteria on 750 E 4th St, Franklin, OH 45005. The purpose of this meeting is to hold an executive session to discuss personnel, employment matters, and the potential sale of property. No action is anticipated.

Final Forms (Physical Information)

We are very excited to announce that the Franklin City Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You will be able to review your information once per school year to verify information is current and sign the forms.

We are asking that ALL parents of athletes use FinalForms. Please register at --https://franklincity-oh.finalforms.com-- now and electronically complete your physical form before Thursday. You will print it off when you have done so and bring it with you to your child’s physical. You can do the other forms at a later date before the sports seasons. You will need your child's email address to complete the physical form!

Health History and Medical Profile
Medications
Injuries and Hospitalizations
Past and ongoing Health Concerns
OHSAA PPE Physician Questions
OHSAA Authorization
OHSAA PPE Physical

*Once you complete OHSAA PPE Physical, click the blue link to print what you will need to bring to your physical.
